We are seeking a dedicated and compassionate Registered Nurse Practitioner to join our team at Hero Health Practitioners, LLC in Carthage, Missouri. As a valued member of our healthcare team,
Registered Nurse Practitioner will have the opportunity to provide high-quality patient care while building meaningful relationships with our residents and patients.

• Provide primary and acute care to patients of all ages, with a focus on family practice

• Conduct thorough patient assessments, develop treatment plans, and prescribe medications as needed

• Collaborate with our experienced physician team to ensure comprehensive patient care

• Stay up-to-date with the latest medical research, technologies, and best practices to provide exceptional care

• Foster a positive and respectful team atmosphere by working collaboratively with healthcare professionals

• Conduct medical exams, take medical histories, and diagnose and treat common illnesses and injuries

• Order and interpret laboratory tests and imaging studies as needed

• Develop and implement individualized treatment plans, taking into account patient needs, preferences, and health status

• Prescribe medications and therapies as needed, and monitor patient response to treatment

• Educate patients and families about healthy habits, disease prevention, and wellness strategies

• Stay current with changes in medical policies, procedures, and regulations

• Two (2) or more years of Nurse Practitioner (NP) experience

• Experience in Family Practice, Adult, Skilled Nursing, Long-Term Care, Geriatrics, Home-Based Primary Care, or Acute Care

• Experience with Point Click Care  or other Long-Term Care EHR systems

• Knowledge of CMS Skilled Nursing Facility regulations

• Familiarity with value-based care models, ACOs, I-SNPs, and geriatric prescribing principles

• Competitive Total Compensation: $125,000-$150,000

• Performance Incentive Program through Hero's Provider Incentive Compensation Plan
