Location: Northern
Caretenders Hospice is seeking an RN Case Manager to provide in-home hospice care. You will deliver the Plan of Care for hospice patients, collaborate with clinicians, and ensure quality of care. On-call evenings and weekends, travel within a 50-mile radius, and standard hours Monday–Friday, 8:00 AM–4:30 PM. Ohio RN licensure is required;
Home-Based Hospice experience preferred. Hourly pay ranges from $43.10 to $64.65.
