Home Health RN Case Manager: Lead Patient Care

Yale New Haven Health at Home seeks a Registered Nurse Case Manager to admit patients for home care services, develop the patient's plan of care, coordinate with providers and other disciplines, and ensure the safe delivery of patient care.

Requirements include a Nursing Diploma or Associate Degree in Nursing, CT RN license (or eligibility), and minimum two years of acute care experience; CPR certification is required. Strong communication and independent work skills are essential.
