Remote RN Care Manager – Transitional Care

Loma Linda University Health is seeking a Care Manager-Registered Nurse (RN) for Transitional Care Management. The role involves leading comprehensive assessments, coordinating medical appointments, therapy sessions, and transportation needs to prevent hospitalizations.

The RN will conduct home visits, follow-ups, and education, working with physicians, pharmacists, and care teams to refine the plan of care. Remote work up to 2 days per week after a 90-day probation period is available.
