You will assess, plan, implement, and evaluate care with the interdisciplinary team, supervise Home Health Aides, and ensure timely, quality patient outcomes in Vermont and New Hampshire.
Requirements include an active RN license in VT/NH or a compact license, graduation from an accredited nursing program, and preferred hospice/homecare experience.
