Health Snap is seeking a Nurse Care Navigator to support NY-based patients enrolled in chronic care management and remote patient monitoring. The role involves educating patients on hypertension, diabetes and other conditions, and coordinating with a patient care team including physicians and health systems.
Hours are Monday–Friday, 9:00am–5:30pm ET. You will build ongoing relationships with patients to empower self-management, document care plans, and participate in quality improvement
