• Ensure seamless care delivery for children with chronic conditions.

Responsibilities:

• Coordinate appointments, referrals, and care transitions.
• Provide patient education and manage medication authorizations.
• Document interactions across EHR and care management platforms.

Qualifications:

• Active California RN license and tech-savvy with EHR systems.
• Strong communication and startup mindset for remote work.
• Bilingual English/Spanish and pediatric experience preferred.
