Community Partnership Manager

COHME is seeking a motivated, relationship-focused professional to help grow community awareness and referral partnerships throughout the New York metropolitan area. This part‑time role is ideal for someone who enjoys networking, building professional relationships, and making a meaningful impact in the lives of older adults and their families.

• Develop and maintain relationships with community organizations, hospitals, and NORCs.
• Conduct outreach through in-person visits, phone calls, emails, and educational presentations.
• Identify and cultivate new referral sources and partnership opportunities.
• Maintain accurate records of outreach activities and contacts in Conversion Copilot CRM.
• Collaborate with leadership to leverage new and existing partnerships and share market insights.
• Attend regular team meetings and provide progress updates.

• Strong communication and relationship‑building skills.
• Comfortable networking with healthcare professionals and community partners.
• Organized, self‑motivated, and able to work independently.
• Experience in healthcare, senior services, home care, sales, marketing, or community outreach is preferred.

• Part‑time, flexible schedule (approximately 15–20 hours per week).
• Combination of community visits, remote work, and occasional meetings.
