• Title: Community Outreach Specialist
• Location: White Plains, Westchester County, NY
• Salary: Up to $100,000 per year
• Local travel within assigned territory required.

• Conduct research to understand local market trends, demographics, and competition to identify opportunities for growth.
• Develop outreach plans and strategies tailored to the target audience.
• Build and maintain relationships with healthcare professionals, community organizations, senior centers, hospitals, and other referral sources.
• Travel within the assigned region to meet with clients and referral partners.
• Organize and participate in community events, health fairs, and seminars to promote home care services.
• Utilize online channels such as social media, email marketing, and website optimization to engage potential clients and referral sources.
• Build rapport with clients and families, addressing questions and concerns.
• Monitor outreach campaign effectiveness, track leads and conversions, and report key performance metrics to management.

• Bachelor’s Degree required;
  Master’s degree preferred.
• Experience with enrollment/intake for clients in a home/community-based setting.
• Knowledge of Medicaid, DSS, DOH rules and regulations, insurance verification processes.
• Experience educating clients on health insurance benefits, entitlements, and assisting with application/enrollment.
• History of developing and maintaining relationships with community partners and referral sources.
• Access to personal transportation and willingness to travel within the assigned territory (75% travel).
