Title: Senior Case Manager
Location: 4822 N Broadway, Chicago, IL 60640
Schedule: Hybrid, Monday to Friday (8:30am-5:00pm)
Base Pay Range: $17.99 per hour - $22.55 per hour

- Provide advanced advocacy, assessment, and case management services to individuals, families, and groups.
- Conduct outreach and intake as assigned.
- Supervise program participants individually or in groups.
- Develop case management plans addressing personal and family adjustments, finances, employment, food, clothing, housing, and physical/mental impairments.
- Offer counseling individually, in families, or in groups; establish service/treatment plans; review progress; and determine case closure in accordance with program guidelines.
- Refer participants to community resources; determine eligibility for financial assistance and benefits.
- Provide financial assistance and management support per organizational policies.
- Assist participants in modifying attitudes and behaviors by increasing self‑awareness.
- Provide case consultation; develop/implement special projects such as informational materials; train staff in social work and service theory.
- Complete forms, case records, and statistical reports accurately and promptly; submit documentation to supervisor within designated timelines.
- Participate actively in supervisory and team meetings, training, conferences, seminars, and independent study.
- Maintain professional relationships with social service organizations and government agencies.
- Adhere to professional standards set by governing bodies, professional associations (e.g., NASW), and funding sources; participate in training and reviews.
- Assist with transportation of participants and/or organizational property as required.
- May be required to maintain household upkeep in residential programs.
- Perform other duties as assigned.
- High school diploma or GED required.
- Minimum seven (7) years of experience in case work, human services, or related field; or Bachelor’s degree plus minimum three (3) years of experience.
- Fluency in a second language (multi‑language ability).
- Ability to read and interpret technical documents and procedure manuals.
- Ability to write routine reports and correspondence.
- Ability to speak effectively before groups.
- Access to reliable transportation.
- Ability to work with diverse socioeconomic backgrounds.
- Excellent communication, networking, and relationship‑building skills.
- Ability to handle confidential information with professionalism and diplomacy.
- High collaboration, emotional intelligence, self‑motivation, and analytical ability.
- Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange, Windows, and other Heartland Alliance software.
- Problem‑solving skills in situations with limited standardization.
- Independent work and self‑motivation with minimal supervision.
- Strategic thought partnership and consultancy skills.
This role is eligible for a hybrid arrangement. The employee will be expected to attend in‑person meetings when needed and may work from home when appropriate.
