Bilingual Case Manager at Heartland Human Care Services

We are looking for a passionate Family Reunification Specialist who is bilingual to join our dedicated team in Chicago. This role has the opportunity to make a meaningful difference in the lives of vulnerable children.

Pay Range

Base pay: $45,256.56/yr – $61,089.00/yr. Actual compensation will be based on skills and experience.

Essential Functions

Duties & Responsibilities
  • Collect and assess family background information through interviews and document review.
  • Refer family reunification cases to partner agencies and recommend appropriate care options.
  • Collaborate with funders and stakeholders and promote best-interest recommendations.
  • Coordinate individual service planning and hold weekly meetings with minors in care.
  • Assist with referrals to foster care and prepare initial referrals.
  • Manage relationships with family members, sponsors, and stakeholders while maintaining professional boundaries.
  • Provide advanced advocacy, assessment, and case management services, and develop service plans.
  • Consult and develop special projects such as informational materials and staff training.
  • Maintain accurate case records and submit reports to supervisors within designated timelines.
  • Participate in staff and working group meetings to support agency strategic plans.
  • Supervise program participants individually or in groups.
  • Help participants modify attitudes and behaviors by increasing self‑awareness.
  • Provide transportation to participants as needed.
  • Assist with household maintenance responsibilities when necessary.
  • Build professional relationships with social service organizations and government agencies.
  • Adhere to professional standards and program guidelines.
  • Engage in periodic evaluations and staff training.
  • Other duties as assigned.
Education and Experience
  • Bachelor’s degree in Human Services, Psychology, Social Work or related discipline.
  • Minimum one year of related experience.
  • Bilingual may be required based on funder requirements.
Certifications/Licenses
  • Driver’s license with no prior suspension may be required.
Knowledge/Skills/Abilities (K/S/A)
  • Ability to speak and read in multiple languages; many languages may be required.
  • Strong written communication skills for reports and correspondence.
  • Ability to speak effectively before groups and stakeholders.
  • Reliable transportation.
  • Comfort working with diverse socioeconomic backgrounds.
  • Excellent communication, networking, and relationship‑building skills.
  • Capability to handle confidential information professionally.
  • High self‑motivation, collaboration, emotional intelligence, and analytical ability.
  • Proficiency in Microsoft Office Suite and related software.
  • Problem‑solving and adaptability in variable situations.
  • Ability to work independently with minimal supervision.
  • Strategic thinking and consultancy skills.
Work Arrangement
  • No remote work; all hours in person at assigned programs.
  • Employees may need to remain on premises during breaks and meal periods until a replacement arrives.
Work Environment
  • Moderate noise level.
  • All program sites operate 24/7, 365 days per year.
  • Flexible scheduling and location required; may be assigned to any site in the Chicago area.
Physical Demands
  • Regular speaking and hearing, sitting, standing, walking, and reaching.
  • Stooping, kneeling, crawling, and lifting up to 25 pounds as required.
