Clinical Case Manager

Title: Clinical Case Manager

Location: 4822 N Broadway Ave, Chicago, IL 60640

Schedule: Hybrid, Monday – Friday 8:30 am‑5:00 pm; evenings and weekends as needed.

Base pay range: $19.79/hr – $24.80/hr (actual pay based on skills and experience).

Summary: Join our team to provide specialized clinical case management services to diverse populations, including children at risk of human trafficking, adults who have experienced trafficking, families, immigrants, refugees, and individuals with mental illness, HIV/AIDS, and/or substance abuse issues. You will independently manage participants, connect them to healthcare, behavioral health, and social services, and develop treatment plans that meet project standards.

Essential Functions

Duties & Responsibilities
  • Provide clinical case management services through a short‑term model, including advocacy, crisis intervention, and referrals.
  • Collect informed consent and non‑clinical assessments.
  • Meet regularly with assigned cases and community representatives to maintain treatment plans.
  • Conduct outreach, intake, create individualized care plans, track community linkages, review case progress, and determine closure in accordance with funder requirements.
  • Develop service plans and screen for income supports and entitlements.
  • Complete all necessary forms, case recordings, and statistical reports promptly.
  • Build and maintain professional relationships with social service, health, and governmental agencies.
  • Participate in supervision, team meetings, in‑house training, conferences, and program/operational planning.
  • Maintain the work volume required by project standards using strengths‑based theory and concepts.
  • Adhere to professional standards set by governmental bodies, NASW, funding sources, and organizational policies.
  • Initiate assistance requests from supervisors for new or complex issues.
  • Perform other duties as assigned.
Education and Experience
  • BA/BSW with 3 years of experience, or Master’s degree in social work, counseling, clinical psychology, or a related field.
  • Bilingual strongly preferred.
Certifications/Licenses
  • LPC/LSW license may be required;
    Certified Domestic Violence Professional preferred.
  • Valid driver’s license may be required.
Knowledge, Skills, and Abilities (K/S/A)
  • Fluency in a second language (e.g., Spanish, Haitian‑Creole) preferred.
  • Ability to read and interpret procedural documents.
  • Skill in writing routine reports and correspondence.
  • Effective communication before groups of customers or organization employees.
  • Reliable transport to main and scattered work sites.
  • Comfortable with people of varying socioeconomic backgrounds.
  • Experience providing trauma‑informed, patient‑centered care.
  • Basic math skills (add, subtract, multiply, divide, calculate ratios and percentages).
  • Excellent communication, networking, and relationship‑building abilities.
  • Confidentiality and professional diplomacy skills.
  • High collaboration, emotional intelligence, self‑motivation, analytical skill.
  • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange, Salesforce, Windows, and other Heartland Human Care Services software.
  • Problem‑solving in variable situations and ability to interpret instructions.
  • Independent work with minimal supervision.
  • Strategic thought partnership and consultancy skills.
Work Arrangement

The role is a hybrid or remote arrangement, primarily home‑based with community engagement. In‑person meetings may be required regularly. Availability for work from home when appropriate is expected.
