Status: Full-Time, Exempt
Reports To: Intact Supervisor

• Develop and implement service plans to support family stabilization
• Conduct weekly home visits and outreach activities
• Facilitate healthy interpersonal relationships within families
• Advocate for clients and assist with school adjustment and permanency planning
• Attend and participate in multi-disciplinary staffings and court hearings
• Maintain accurate and timely case records and progress notes
• Collaborate with community agencies to ensure wraparound services
• Participate in training, seminars, and professional development

• Education: Bachelor’s degree in social work or a related field (Master of Social Work or equivalent preferred)
• Experience: Professional experience in child welfare or family services preferred
• Requirements:
  • Successfully obtain Intact Specialty Certification
  • Valid Illinois driver’s license and proof of required auto insurance
  • Ability to pass a background check
  • Strong communication, organizational, and interpersonal skills
  • Ability to work independently and as part of a team
  • Comfortable with home-based services and on-call duties
