, the Case Manager provides individualized support to clients who are non-traditional students, helping them access services, achieve personal goals, and enhance their overall well-being. This position involves assessing client needs, developing service plans, coordinating resources, monitoring progress, and maintaining accurate documentation. The Case Manager collaborates with internal teams, community organizations, and service providers to ensure comprehensive, client-centered care.
- This is a part-time, hourly, grant-funded position which will end when funding is no longer supported.
- This remote position will require being on campus for meetings, orientations, and meetings with students as needed.
Duties will fall under one or more of the following initiatives:
25% Provide intake, assessment, and eligibility determination for individuals.
5% Coordinate and conduct orientation presentations regarding the training program and expectations to potential clients/students.
10% Develop individualized career plans tailored to each client's goals and needs.
20% Offer ongoing case management, including regular follow-ups, progress monitoring, and meeting outcomes.
15% Advocate for clients and address client barriers through supportive services. Make referrals and connect clients to essential services, including housing, healthcare, childcare, and other community resources.
20% Maintain accurate and timely documentation of client interactions, services provided, and outcomes in the case management system. Ensure program compliance with grant requirements, data collection standards, and reporting procedures. Maintain confidentiality and uphold ethical and professional standards in all client interactions.
5% Participate in team meetings with staff, instructors, and other relevant personnel.
Knowledge, Skills, and Abilities- Knowledgeable regarding federal and state regulations regarding allowable expenses.
- Proficient in computer technology relative to the position.
- Ability to direct and support others to strive for individual goals.
- Excellent communication, organization, and problem-solving skills.
Preferred Qualifications Required
Education and Experience
- Bachelor’s degree.
- At least 2 years of experience in case management, social services, or a related area.
- Experience working with diverse, underrepresented individuals facing barriers.
- Knowledge of local social service systems and community resources.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and collaboratively in a demanding environment.
- Proficiency in using case management software and Microsoft Office applications.
Education and Experience
- Bachelor’s degree in social work, Human Services, Psychology, Counseling, or related field (Master’s preferred).
- Knowledge of apprenticeship/work-based learning programs
