** Your Role as a Support Coordinator
** As a Support Coordinator, you’ll work directly with Medicaid participants in self-directed care programs.
Your role is to educate, guide, and support them in managing their care plans, hiring direct care providers, and ensuring their home environment is safe.
You'll be out in the field meeting participants in their homes for at least 60% of your work hours, ensuring they have the resources they need to remain independent. The rest of your time will be spent completing documentation and connecting with team members from your home office.

* What You’ll Do:

* ** Visit participants in their homes** (travel up to 60 miles) to provide guidance and education.
* ** Assess home environments
** and help resolve any safety concerns.
* ** Empower participants
** by helping them understand and manage their employer responsibilities.
* ** Organize and manage your caseload efficiently
** to maximize your impact.
* ** Collaborate with a dedicated team
** while working independently in the field.
* ** Use technology tools
** like Microsoft Office, CRM platforms, and electronic records to stay organized.
** What You Bring to the Table:
** Passion for helping others live independently Strong communication & interpersonal skills Ability to multi-task, prioritize, and stay organized
Experience with Microsoft Office and comfort with digital tools A valid driver’s license, a reliable vehicle, and a secure internet connection
** Preferred:
** A degree in social work, psychology, or a related field and 1+ years of experience working with vulnerable populations.
