If you have a bachelor's in social work or a related field, are licensed in Social Work or seeking licensure, this is a great opportunity for you. Our work environment is highly collaborative and service‑oriented, serving individuals in the seven‑county metro area.

Case Managers have a great deal of independence while managing their caseloads, working with individuals on CADI and AC/EW waivers. These individuals may have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability, or be aging.

• Assess the areas of the individual's life where assistance is needed.
• Offer and coordinate resources for the individuals supported.
• Monitor service delivery and work with providers to ensure needs are met.
• Develop service plans.
• Attend regular in‑person meetings with individuals and their interdisciplinary teams.
• Case‑note related work to create billable units.
• Attend two‑thirds of client meetings in Hennepin County and one‑third in Scott County.

• Graduate from an accredited four‑year college with a major in social work, psychology, sociology, or a closely related field, OR
• Graduate from an accredited four‑year college in any field with at least one year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.
• Valid driver's license with an acceptable driving record and vehicle with current insurance.
• Successful completion of local and/or state background checks.

• Possess an LSW or higher BOSW licensure.

Non‑LSW: $49,000/year (salaried for 40 hrs/week). LSW: $51,600/year (salaried for 40 hrs/week).
