Job Description

The Old Colony Y Intensive Home based Therapeutic Care Program is seeking energetic and flexible outreach staff who have a passion to make a difference for children and families in southeastern MASS communities. The Outreach Caseworker will be part of the Intensive Home Based team working with an assigned caseload of youth and families referred by the Department of Mental Health.

The Outreach Caseworker will be comfortable working both independently and collaborating with the team in supporting youth experiencing mental health needs that impact their home and community relationships. This role involves various treatment oriented services, family engagement, advocacy, life skills coaching, and connecting youth and families to local community resources. This role also involves crisis interventions to support families, participation in family team meetings, care coordination, treatment documentation, and group staff development meetings.

Requirements
  • Bachelor's degree in Social Work or related field, and at least 1 yr. of direct care experience gained through part or full-time paid work, volunteer, or internship experience, or currently enrolled in bachelor's degree program in Human Services with extensive work experience with individuals or families experiencing mental illness.
  • Possess a current valid driver's license and registered and insured automobile. Must be at least 21 years of age and able to drive daily to transport youth in the community, to attend youth treatment meetings, home visits, etc.
  • Excellent communication (written and verbal), organizational, and interpersonal skills.
  • Possess basic computer skills to complete treatment documentation and data entry.
  • Ability to work a flexible schedule inclusive of days, evenings, and weekends.
  • Ability to work independently and perform the majority of work tasks in the community (including but not limited to the homes, schools, hospitals, respite, or other out of home placements for assigned youth cases.
  • Ability to also work collaboratively as part of a therapeutic team, and providing weekly communication with Clinical Care Coordinator in delivering supportive family engagement services to assist youth and family with treatment plan goals.
