What You'll Do

As a Youth & Family Support Specialist, you'll partner with a Master's Level Home-Based Clinician to help children, youth, and families who are at risk and/or have a history of psychiatric or substance use disorders. This role exists to strengthen engagement, improve follow-through on care plans, and connect families to the right supports.

  • Provide clinical support and case management
  • Coordinate with referral sources (including DCF, DMH, schools, and other providers)
  • Document and track service needs
  • Collaborate with internal teams to keep care organized and responsive

Required qualifications/skills:

  • Experience working with youth and families; strong family engagement approach
  • Knowledge of community resources and systems (DCF, DMH, DYS, DDS, Courts)
  • Cultural competence across cultures/ethnicities; bilingual Portuguese/Cape Verdean Creole strongly preferred
  • Team-oriented, highly organized, strong time management
  • Valid driver's license and reliable vehicle; able to lift/carry 10–25 lbs and navigate stairs
  • Bachelor's/Associate's in Human Services or HS diploma with commensurate experience; 12+ months preferred

A Typical Day

Your day as a Youth & Family Support Specialist starts by reviewing referrals and coordinating next steps with your Clinical Supervisor and the home-based clinician. You'll spend much of the day in the community—traveling to meet families in person, adjusting plans when schedules change, and staying reachable for quick check-ins.

Between visits, you'll connect with schools, state agencies, and other providers to align services, then return calls and messages so families aren't left waiting. You'll keep notes and records current, flag urgent needs to the team, and make sure handoffs are clear.

By late afternoon, you'll regroup with colleagues to confirm upcoming appointments, troubleshoot barriers (transportation, communication, system navigation), and set priorities for the next day—ending with a plan that keeps families moving forward.
