Overview

Up to $1000 New Hire Sign-on Bonus

Pay: $32 an hour

Provides coordination in all aspects of home-based therapeutic service treatment plans and P.A.S.S. plans for specified caseload in compliance with EOHHS & MCO standards. Provides general training to staff and families related to policies, procedures, forms, etc. as related to the HBTS and P.A.S.S. programs. Provides ongoing communication via telephone calls, documentation and attendance at meetings with all relevant parties involved with the specific child.

Schedules and facilitates team meetings as applicable to include family members, therapists, consultants, clinical supervisor, school personnel and CEDARR clinician or family service coordinator as applicable. Ensures child and family centered involvement throughout all aspects of plan development, treatment and follow-up.

Responsibilities

Essential Functions

Communicates with members supported, staff, and other stakeholders in both languages.

Initiates and coordinates Home Based Therapeutic Services and Personal Assistance Service and Supports for eligible children and families via the EOHHS & MCO'S.Conducts initial interviews with families to identify specific needs/target areas and problematic times for scheduling purposes.

Obtains all relevant information from various physicians, school departments, therapists and others via signed release forms.

Completes initial proposal packet and assists clinical supervisor and/or treatment consultant in plan development, approval process and all coordination and follow-up.

Assists families with techniques for identification and recruitment of workers when necessary.

Schedules interviews with potential workers and families upon approval of treatment plans.

Provides training and performance evaluation with staff and families regarding department policies and procedures.

Conducts activities on behalf of a specific child to ensure coordination with all relevant caregivers and others involved in the child's plan of care including periodic home visits, functional assessments and other activities.

Tracks data and progress notes in a timely manner with clinicians.

Completes renewal proposals with consultant or clinical supervisor.

Advocates on behalf of a specific child to ensure coordination with all relevant caregivers and others involved in the child's plan of care including periodic home visits, functional assessments and other activities.

Reviews and processes incident reports per agency policy

Attends meetings or appointments with outside parties as applicable.

Provides all required communication and documentation to EOHHS & MCO'S pertaining to the specific caseload.

Informs families of agency mission, policies and procedures such as the bill of rights, grievance procedures and transportation policy.

Provides resources to families regarding various services, transition planning and available information.

Assists with transition process when the child is exiting the program due to age or other reasons.

Completes all discharge forms in conjunction with EOHHS & MCO'S as applicable.

Attends IEP meetings as requested by parent or in place of assigned clinician.

Contributes to the positive morale by representing the agency through professional communication and interactions.

Implements and fosters the principles of self-advocacy which promote an independent lifestyle and respect for program participants.

Assists people with learning about and advocating for their personal rights and understanding the responsibilities that accompany those rights.

Follows all established policies and procedures and documents accordingly.

Drives people to destinations necessary for them to experience a full and diversified life, following all safety and legal regulations.

Additional Responsibilities

Arrives punctually and participates in all required meetings and trainings.

Provides support to other areas of the agency, as needed.

Complete and maintain all required trainings, certifications and licensures.

Safeguard sensitive and confidential data.

Actively promote and positively role model philosophy, mission, and core values of the organization. Treat individuals served, associates, guests and other stakeholders in a respectful and dignified manner.

Respond effectively to potential safety hazards and emergencies. Maintain a clean, orderly and safe environment by being respectful of property and equipment. Follow proper maintenance and safety  familiar with and actively promote all Seven Hills' policies and procedures, the site's program specific policies and procedures and the regulations governing the program.

Other duties as assigned.

Qualifications

Bachelor's degree from a four year college or university. One to two years related experience and/or training in a social service setting.

Valid Driver's License, ability to drive and means of transportation. Travel to other cities and towns may be required.
