ABOUT PATH - Since its foundation, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting clients to a continuum of homelessness prevention, street outreach, employment preparation and placement, individualized case management, supportive services, interim housing, and permanent supportive housing.

About the JobAs part of the Housing Navigation Team
, the Case Manager/Housing Navigator helps clients break the cycle of homelessness using a “whatever it takes” approach.
Position Responsibilities
- Conduct intake and individualized needs assessments for all clients and collaborate to develop an Individualized Service Plan (ISP) addressing housing goals, income goals, medical, mental health, substance use, financial resources, vocational, and social support needs.
- Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals.
- Complete program-specific assessments (e.g., VI-SPDAT).
- Evaluate strengths and challenges to address short-term and long-term goals; conduct a check-in after initial intake.
- Maintain documentation standards as required by the Program and PATH policies; complete case file notes in a timely manner and maintain a complete, accurate, and current client file with all required documents.
- Maintain confidentiality of client files per HIPAA and applicable guidelines.
We’re looking for candidates with:

Analyze information, identify key issues, consider alternatives, and make sound decisions. - Ethics and Boundaries:
Maintain ethical standards, professionalism, and boundaries.
- Two (2) years of experience in healthcare, case management, homeless services, or related fields.
- All levels of education and experience are welcome.
Regardless of qualifications, candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Successfully complete:
Tuberculosis Test, Background Screening, Drug Test
Driving is an essential function of this position. To meet employment requirements, all candidates must:
- Have reliable transportation
- Have a valid driver’s license
- Provide proof of insurance and ownership for personal vehicles used during work duties
- Be able to qualify for PATH's insurance coverage
