, Adrian MI – is hiring a full‑time Case Manager – Home Based (Children’s Services). This onsite position requires travel to the consumer’s location and involves intensive services to children and families, focusing on the family unit.

Starting salary: $1,038.00 per week (annual range $53,976 – $69,810 based on years of service). The position offers paid leave, holidays, health, dental, vision, retirement, and disability benefits, and is eligible for loan repayment programs.

You assist eligible minor consumers (and their families) with the development of a youth‑driven / family‑guided Person / Family‑Centered Plan (P/FCP) emphasizing strength‑based, culturally relevant interventions, parent/youth and professional teamwork, and connection with community resources and supports. You work with the minor consumer (and their families or chosen support network) in developing a P/FCP that promotes individual choice, the use of community resources, and formal and informal supports in pursuit of the minor consumer’s desired goals.

You monitor the supports and services according to the plan and effectively track progress toward the minor consumer’s desired outcomes.

• Bachelor’s degree in a human services field from an accredited program.
• Licensed by the State of Michigan to practice in a human services field (if applicable).
• Three years supervised work experience in examination, evaluation, and treatment of children with serious emotional disturbances.
• Possess a valid U.S. driver’s license.

• Paid leave accruing from first day; approximately 24.5 days off in the first year and increases after one, five, and ten years.
