Position: Aging Care Manager 3 (Local Government) - Delaware County Area Agency on Aging

Delaware County Area Agency on Aging is searching for an Aging Care Manager 3 to work with individuals, families, and community partners to ensure safety and well‑being. This position allows you to build strong relationships while responding to important needs in the community, offering support, guidance, and coordination for essential services.

DESCRIPTION OF WORK
  • Risk Assessment:
    Gather information from consumers, caregivers, and others to determine the level of risk and whether reports are substantiated.
  • Care Planning:
    Develop plans that address concerns, arrange supports, and coordinate needed services to reduce risks.
  • Case Documentation:
    Maintain detailed records of contacts, observations, and case activity in the required system.
  • Home Visits:
    Conduct face‑to‑face visits to monitor safety, review concerns, and speak with individuals involved in the case.
  • Emergency Response:
    Arrange shelter, initiate involuntary procedures, or seek guardianship when necessary to protect consumers.
  • Court Preparation:
    Prepare documents for court proceedings and provide testimony when required.
WORK SCHEDULE AND ADDITIONAL INFORMATION
  • Full‑time employment.
  • Work hours are 8:30 AM to 4:30 PM, Monday – Friday, with a 30‑minute lunch.
  • Travel, overtime, and after‑hour duties may be required.
  • Telework:
    Opportunity to work from home part‑time after six months of employment, provided you have a securely configured high‑speed internet connection and work from an approved location inside Pennsylvania. If unable to telework, you may report to the headquarters office in Eddystone. Telework eligibility is subject to change.
  • Salary:
    In some cases, the starting salary may be non‑negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS
  • One year as an Aging Care Manager 2 or Aging Program Assessor; or
  • One and one‑half years of professional human services, law enforcement, or nursing experience, and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training that includes six months of professional human services, law enforcement, or nursing experience.
OTHER REQUIREMENTS
  • Possession of a valid driver’s license not under suspension.

LEGAL REQUIREMENTS
  • Pass a background investigation.
  • Fall under the provisions of the Older Adult Protective Services Act. Under this Act, a conditional offer of employment requires submission and approval of satisfactory criminal history reports, including but not limited to PA State Police and FBI clearance.
  • Fall under the provisions of the Child Protective Services Law. Under this law, a conditional offer of employment requires submission and approval of satisfactory criminal history reports, including but not limited to PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
