Case Manager - Parenting and Family Support Who We Are

What is the purpose of our Supported Parenting Program?

The Supported Parenting Program is a family support, home based service program designed to prepare, strengthen, and support parents who are navigating challenging life circumstances. Families we serve are often adjusting to new parenthood, building routines, supporting their child’s development, and balancing the many demands that come with raising a young child. This program promotes positive parenting practices and healthy parent‑child interactions in everyday life.

Our goal is to empower parents to meet their children’s developmental needs, reduce barriers to stability, and foster long‑term family well‑being.

Supported Parenting Services

What Will I Do as a Family Support Case Manager?

In this role, you will:

  • Provide in‑home visiting services to parent(s), first time parent(s), and those with children who may benefit from additional support to promote positive outcomes.
  • Create individualized plans to help parent(s) develop confidence in caring for their children and building self‑esteem.
  • Provide education about child growth and development as well as offer support and reinforcement for healthy parenting skills.
  • Offer assistance to all members of the family to resolve the tensions and discord which can arise about co‑parenting, visitation, discipline, etc.
Qualifications
  • Bachelor’s Degree in Social Work, or related field required, such as Human Development and Family Studies, Human Services, or Sociology.
  • At least six months experience in a social service or health care setting required; 2+ years preferred.
  • Valid driver’s license, reliable transportation, and vehicle insurance (liability coverage: $100,000/person and $300,000/accident).
Salary Range

$45,000 - $47,000
