Location: Indianapolis
Responsible for providing a continuum of social work services in an assigned work setting that will assist patients/clients and their families find practical ways of resolving social, emotional and environmental problems that interfere with optimum functioning and attainment of total health. Works closely with other community agencies and service providers to maximize health outcomes for individuals, families, and communities. The majority of services provided are home-based.
Essential Duties- Provides therapeutic social work intervention to diverse client system which may include but not limited to crisis intervention, community resource referrals, client advocacy, support, outreach, follow-up, education, and intra/inter agency consultation.
- Participates in a multidisciplinary team effort to coordinate/implement services that will ensure appropriate treatment plan continuity of care for all clients/patients served of care.
- Serves as social work services resource/consultant person concerning appropriate management of social situations that affect the effective functioning of individuals and families.
- Prepares written reports and clinical records (progress notes, summaries, statistical forms, etc.) of services rendered; responds timely to requests for information.
- Assumes advocacy/liaison/collaborative role for patients/clients via contacts with community agencies/personnel, hospital personnel, court disposition hearings, abuse cases, etc.; collaborates with inter/intra agency personnel involving referral activities.
- Maintains a current file of referral sources; cultivates a network of resources and collaborates with inter/intra agency personnel involving referral activities.
- Patient/Client and their family’s records and information, staff records and/or their families, of all other confidential information presented orally and or in writing will be kept confidential.
- Maintain confidentiality of patient/client records and information in accordance with HIPAA guidelines.
- Assumes responsibility for maintaining current knowledge of information relevant to specialty area.
- Functions as a skilled consultant concerning effective management of interpersonal situations that affect the positive coping skills of individuals, families, at-risk groups.
- Documents services rendered via client records, statistical records, and written summaries.
- Observes and Interprets impact on client functioning in environmental conditions and family interactions through home visits, office visits and community encounters.
- Assists the Administrator of Social Work and Clinical Coordinator in program design and implementation of social work services, goals, and objectives.
- Utilizes communication and problem-solving knowledge/skills to identify and facilitate resolution of problems in team functioning.
- Participates in staff meetings, in-services, educational programs, and trainings; and assumes responsibility for continuous professional self-development.
- Assists in developing and leading groups in the community, based on identified needs and evaluates the group’s effectiveness.
- Participates on SW/ MCPHD & HHC/Community committees/projects as required or assigned.
- Frequent external contacts with patients/clients, families, health, and social service agencies concerning referrals, care coordination and acquisition/provision of confidential information.
- Occasional requirement to testify in court cases (abuse/neglect, custody).
- Perform other duties as assigned.

Duties and Responsibilities:
- Serve on appropriate committee and task forces as assigned.
- Trained at the appropriate National Incident Management System (NIMS) and Incident Command System.
- Training on Psychological First Aid (PSA) in preparation to participate in the response to public health disasters/emergencies.
- Participate in Health Fairs.
- Minimum:
Bachelor’s Degree in Social Work. - Valid Indiana Social Work License
- A valid Indiana Driver’s License and access to a vehicle is required.
