Major Responsibilities/Activities
- Provides home based services to families in Erie County utilizing the Mii Wrap Care Coordination model
- Utilizes a strengths-based family driven approach to services
- Partners with children, parents, guardians, Erie County Department of Social Services and service providers to create a comprehensive Plan of Care and identify any additional services the family may benefit from
- Conducts assessments throughout duration of the case
- Provides education, care coordination, and community linkages as appropriate
- Conducts face to face contacts with families (in home and community)
- Must be willing and able to travel through all of Erie County
- Conducts Team meetings each month
- Participates in trainings required by stakeholders and agency
- Miiwrap credentialling is required
- Will receive evidence based training to support the practice
- Supervisors will credential all staff in Miiwrap
- Maintains timely and accurate documentation in two systems
- Other duties as deemed appropriate
- Solid writing and verbal communication skills
- Strong engagement skills and ability to engage with diverse populations
- Adaptability when working with families and prioritization of tasks
- Flexibility related to scheduling (nontraditional work hours)
- Mandatory knowledge with computer applications (i.e. Word, Excel, Outlook, Edge)
- Familiarity with Fidelity EHR and Connections are a plus
- Bi-lingual ability is a plus
- Motivational Interviewing skills are a plus
- Bachelor’s Degree or Master’s Degree in a human services related field plus 1 year experience in a professional or internship human services setting.
* Experience must be providing direct care services or linkage services to at risk youth/children and families. - Valid NYS Driver’s License and adequate auto insurance
- Ability to work effectively with clients, families, staff and community contacts from a variety of cultural and ethnic backgrounds.

Competitive Pay Range$48,000- $52,500 annually based on experience and qualifications
