Contract Care Coordinator Assessor, LSW or RN (Part-time or Full-time)

Our multidisciplinary care teams collaborate with patients, their families and other healthcare providers to deliver care that is preventative, proactive and tailored to their unique needs.
Position Overview:
Work from home, with approximately 30% travel within Hennepin County, MN. Contractor roles may be part‑time or full‑time, with flexible scheduling within regular business hours.
Part‑time or full‑time contractor position with adaptable work hours.
LocationAbout 70% remote work from home and 30% onsite travel to Hennepin County, MN (mainly Minneapolis and St. Paul). Mileage reimbursed per IRS guidelines.
Pay Range$35 per assessment; hourly rate adjusted based on experience.
Duration of AssignmentASAP until September/October 2026, with potential to transition from Temp to Full‑time.
BenefitsNot benefits eligible as a contract employee.
Responsibilities- Contact new enrollees to complete the initial assessment (full‑time contractors expected to complete approximately 8 assessments per week).
- Review current health needs, identify goals, and develop individualized care plans and transitions of care.
- Assist members in connecting with resources and services.
- Complete required documentation.
- MNChoices experience highly preferred (paid training available).
- MN Licensed Social Worker (new graduates encouraged).
- Current Minnesota Registered Nurse license.
- At least one year of experience working with geriatric or certified disabled populations in case management/care coordination, home care, group homes, nursing homes, TCU, or assisted living settings preferred.
- Valid driver’s license.
- Ability to work independently.
- Access to a private and compliant home office space.

- Excellent interpersonal and customer service skills.

- Adequate reading, writing, speaking, and understanding of English.
