Program Overview – Case Management, Homeward Bound

Homeward Bound is an employment wrap‑around program that assists female‑led families who are living in unstable conditions to make a successful transition to sustainable employment, permanent housing and independent living for themselves and their children. The program includes many components including career development and life skills, academic upgrading and computer training, a college diploma program, an internship component, and access to sustainable employment opportunities.

Homeward Bound also offers on‑site childcare, family support, and an after‑4 program for school‑aged children.

This role also involves providing crisis management, continuous support, and advocacy for participants and their families, working closely with the Toronto Community Housing partners and other team members to foster a stable and supportive community environment.

What You Will Do
  • Intake Process Involvement:
    Support with the coordination of the entire intake process for new participants, including conducting behavioral interviews, ensuring the timely completion of background checks, and managing all other steps of the selection process.
  • Case Management:
    Utilize professional case management practices and various clinical modalities to perform assessments, set goals, and develop action plans. Provide ongoing intervention, re‑assessment, and follow‑up, culminating in case termination when appropriate.
  • Program Coordination:
    Oversee participant involvement in all aspects of the Homeward Bound program, including liaising with community colleges, academic and upgrading providers, and coordinating on‑job training experiences such as field placements and internships.
  • Liaison with Social Services:
    Maintain ongoing communication and coordination with Toronto Children Services and Ontario Works.
  • Support Services:
    Provide practical assistance to participants in resolving daily living challenges, advocating for them, and facilitating access to essential services such as healthcare, financial management, and childcare support.
  • Referral and Monitoring:
    Identify learning barriers and refer participants to appropriate support services. Liaise with community agencies and monitor participant progress, particularly for those with complex socio‑emotional needs.
  • Crisis Intervention:
    Respond to emergencies and provide crisis intervention while managing an assigned caseload.
  • Record Keeping:
    Maintain accurate client records in a centralized database in accordance with program policies.
  • Data Collection:
    Collect and report statistical data as required by center and unit policies.
  • Community Liaison:
    Attend meetings with community agencies as recommended by the Manager.

    Participate in Homeward Bound team meetings, case management meetings, and work effectively with both program and unit teams.
  • Professional Development:
    Engage in training and education as directed by Wood Green Community Services to enhance job performance.
  • Anti‑Racism and Community Development:
    Ensure that the program aligns with the organization’s anti‑racism objectives and community development principles.
  • Committee Representation:
    Represent the unit on internal staff committees as directed by the Unit Director.
  • Additional Duties:
    Perform other related duties as required and participate in regular supervisory sessions with the Manager.
  • Site Flexibility:
    Be prepared to relocate to another site within Wood Green’s multi‑site locations if program needs dictate. Reasonable notice will be provided.
What You Bring to the Team
  • Bachelor’s degree in Social Work or a related field.
  • Two years of related work experience with women who have experienced homelessness, violence, or similar challenges, particularly at a case management level.
