Responsibilities
Provide case management to individuals with developmental delays, mental health needs, or intellectual disabilities. Monitor, link, and evaluate services; make referrals and provide crisis intervention as needed. Maintain regular direct contact and conduct face‑to‑face case management. Facilitate meetings to develop individualized service plans in collaboration with the client and/or family. Record and update information in the agency’s computerized database and maintain compliance with state, federal, and licensing regulations.
Attend staff meetings, trainings, and provide coverage as required. Work in collaboration with supervisors, other providers, and community resources.
- Full‑time employment, 8:00 AM–4:00 PM, Monday–Friday, with a 30‑minute lunch break.
- Occasional work outside of regular hours to meet project deadlines or emergencies.
- May provide coverage in Somerset County as needed.
- Upon hire staff begin a 6‑month probation period; after 3 months they may be eligible for a hybrid of on‑site and remote work.
- Six months as a County Caseworker 1 or
- Successful completion of the County Social Casework Intern program or
- A bachelor’s degree with a social welfare major or
- A bachelor’s degree supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences and one year of professional social casework experience, or
- Any equivalent combination of experience and training that includes 12 college credits in related social sciences.
Applicants must be within two months of graduating with a qualifying degree or credits.
Other Requirements- PA residency requirement is currently waived for this title.
- You must be able to perform essential job functions.
- Background investigation is required.
- This position falls under the Child Protective Services Law and the Older Adults Protective Services Act, requiring satisfactory criminal history reports.
