Community Housing Coordinator

Sigma Systems Inc. is seeking a Community Housing Coordinator on behalf of our client based in Pittsburgh, PA. This position offers the flexibility of remote work combined with meaningful field engagement supporting at-risk populations.

Position Overview:

Community Housing Coordinator Sigma Systems Inc. is hiring a Community Housing Coordinator to support members in identifying and securing affordable, accessible, and sustainable housing options. This role requires a balance of remote work and local travel, engaging directly with members and community partners to remove barriers and improve housing stability.

Key Responsibilities:

  • Housing Coordination and Member Support:
    Research, evaluate, and analyze demographic data to identify viable housing and utility support options. Identify member barriers and develop action plans to guide members toward sustainable housing solutions. Engage in empowering interactions to support member independence and stability.
  • Community Engagement and Outreach:
    Meet with health plan members face-to-face to support their housing needs. Represent the organization at community events and partner-sponsored initiatives. Build and maintain strong relationships with local housing agencies and organizations.
  • Collaboration and Liaison

    Activities:

    Act as a liaison between members, Case Managers, and housing agencies. Collaborate with federal, state, and local agencies (including HUD and housing authorities). Support coordinated efforts to connect members with appropriate resources.
  • Documentation and Compliance:
    Utilize the health plan's system of record to document all member interactions. Maintain accurate and timely documentation in compliance with organizational standards.
  • Program Development and Continuous Improvement:
    Participate in developing innovative and measurable programs focused on at-risk populations. Serve as a subject matter expert in affordable housing options. Identify opportunities to improve housing support services and member outcomes.

Qualifications:

  • Education:

    High School Diploma or GED required
  • Experience:

    1 year of experience in community outreach or customer service. 1 year working with at-risk populations (preferred). Experience with Medicaid regulations. Experience in healthcare customer/member services. Experience working with unhoused populations
  • Skills:

    Strong knowledge of customer service principles, including needs assessment and quality standards. Ability to work collaboratively with peers, supervisors, and customers. Strong problem-solving skills with a proactive and self-directed approach. Ability to multitask and respond promptly to member needs. Excellent written and verbal communication skills. Cultural competency and sensitivity toward Medicaid populations, including those with limited English proficiency. Understanding of barriers such as transportation, childcare, and healthcare access.

    Demonstrated patience and empathy in all interactions. Ability to establish relationships with housing authorities and agencies. Knowledge of federal and state housing laws, Medicaid programs, and benefit packages

Work Schedule & Conditions:
Remote position with local travel required up to 75% (travel is reimbursable). Must reside within one of the following Southwest PA counties:
Allegheny, Armstrong, Beaver, Bedford, Blair, Butler, Cambria, Fayette, Greene, Indiana, Lawrence, Somerset, Washington, Westmoreland. Schedule includes regular business hours Monday – Friday with occasional evening or weekend availability for community events
