About

The Role

Perform skill building functions that support the development of a child/teen and adults’ life and social skills, working one on one with each participant in his/her own home and in the community. Facilitate family members’ understanding of the child/teens and adults’ needs as related to his/her condition/diagnosis.

Essential Duties and Responsibilities
  • Provide a meaningful “core” service for each child/teen and adult assigned, including face-to-face visits per month based on need and acuity level, to ensure all care plan goals are addressed to the satisfaction of the clients served.
  • Work directly with families to help them understand their child’s condition and subsequent needs, and ensure adult needs and understanding of conditions are met.
  • Demonstrate emotional confidence and good judgement to engage children, families and adults.
  • Manage complex behavioral needs of children while facilitating skill building activities, ensuring safety and applying appropriate emergency and crisis intervention principles and techniques.
  • Provide prevocational services including teaching skills, building appropriate work habits, completing applications, role playing interviews, etc., as needed.
  • Provide various supported employment services to assist youth and adults with maintaining employment, including on-the-job support.
  • Enter required documentation of every interaction with client into Electronic Health Record system to meet contract expectations.
  • Provide LPHA-driven and Family and Youth informed skill building activities to caseload assigned, maintaining accurate documentation of all services rendered.
  • Using a person-centered approach, complete comprehensive assessments, reviews, crisis plans, and care plans as required by standards set by DOH and lead Health Home contractors.
  • Demonstrate, promote, and practice cultural competency towards clients through respect and understanding.
  • Recognize and embrace diversity by supporting and encouraging the strengths of each individual youth and adults served.
  • Assist youth and adults in the acquisition and maintenance of public benefits, if applicable.
  • Provide linkages/referrals and coordinate resources/support for medical care, mental health, social, recreational, and housing/basic needs.
  • Remain current in the field, support and implement best practice service models within the program.
  • Make independent decisions and recommendations regarding immediate risk factors presented and provide crisis intervention and follow-up as needed.
  • Provide education to youth and family concerning the nature and desired effect of services.
  • Assist youth and families in managing and planning for safety needs by implementing individualized safety/support plans.
  • Maintain accessibility for clients during the workday, demonstrating flexibility to be available outside normal business hours as needed.
  • Serve as client advocate.
  • Participate as a member of the youth’s Care Team to carry out individualized and strength-based plan of care through regular case conferences and case reviews.
  • Participate in staff meetings and other CFTSS/HCBS department activities.
  • Ensure compliance with all applicable regulations and agency policies.
  • Participate in mandatory ongoing training and professional development offered.
  • Represent Catholic Charities on appropriate boards and committees as assigned by CC Director.
  • Other duties as assigned.
