This isn't just a job—it's your chance to empower individuals with developmental disabilities and their families across Sullivan and Lower Grafton County, NH, helping them live their very best lives.

What

You'll Do

As a Family Support Coordinator, you will be the ultimate bridge to vital resources, community connection, and advocacy:

  • Advocate & Empower:
Connect families to state and community resources while maximizing their control over their own services.
  • Guide Transitions:
Help individuals smoothly plan and navigate the massive milestone of transitioning from school to adult services.
  • Navigate Legal & Financial Pathways:
Assist with budget administration and facilitate important processes like Guardianship and Power of Attorney.
  • Collaborate & Document:
Facilitate team planning meetings and maintain meticulous documentation alongside a passionate, friendly team.
What We're Looking For

We need a flexible, creative, and non-judgmental professional who genuinely loves serving diverse families.

  • Education:

A Bachelor's degree is highly preferred, but extensive case management experience will absolutely be considered instead.
  • Experience:

At least 2 years in human services, with a minimum of 1 year in family support. Experience with developmental disabilities is a huge plus!
  • Skills:

Excellent communication, strong organization, intermediate MS Office proficiency, and a willingness to learn new programs.
  • Travel:
Reliable transportation for regional/statewide home visits and meetings.
What We Offer
  • Competitive Pay: $22.00 to $25.00 per hour, depending on experience.
  • Perks: A generous hiring bonus for all new employees!
  • Flexibility: A hybrid work-from-home model may be available after your first 90 days.
  • Robust Benefits:
Health, dental, vision, company-paid life/disability insurance, and a 403(b) retirement plan.
  • Balance:
Generous paid time off (PTO) and paid holidays in a truly supportive, friendly work environment.

If you are ready to make a lasting impact and help individuals with developmental disabilities live their best lives, we want to hear from you! We will train the right person, so if you have the passion, we encourage you to apply.

We are an EEO employer and welcome people of diverse backgrounds and experience.
