The case manager position is designed to assist persons served in identifying and acquiring both natural and paid supports that enable them to live as independently as possible in the community. The case manager should use a person‑centered approach to support individuals in making their own choices and decisions about these supports, functioning as an educator, resource, and service coordinator. Life decisions should be made by the individual and/or guardian.
The case manager will coordinate services for vulnerable individuals, navigating the complex service delivery system. Responsibilities include an ongoing assessment, planning, referral, service coordination, monitoring, consultation, and advocacy to address multiple service needs. The position involves directly managing a caseload and facilitating coordination of additional services or supports that positively influence individuals’ lives.
The case manager will maintain contact with a wide range of organizations, families, and community members, acting professionally at all times and representing the interests of the individual they support.
Work Environment and HoursThis position requires local travel on a weekly basis. A hybrid remote work environment is available, but applicants should expect to travel regularly throughout the work week to meet the needs of the clients served.
Probationary PeriodDuring the first 4‑6 months, all work will be in‑person Monday to Friday.
Qualifications- Bachelor’s degree in social work, psychology, sociology, or a closely related field, or a license as a social worker.
- At least one year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.
- A valid driver’s license with an acceptable driving record.
- Proof of automobile insurance.
- Knowledge of the local community.
- Strong oral and written communication skills.
- Understanding of concepts related to self‑directed services.
This position is an unlimited driving role: the employee will be required to operate a vehicle for Dungarvin work, including vehicles owned or leased by the organization, and/or transport individuals served.
