Position: Social Worker (Lead Team Case Manager) / 60019564

This position is located in Community Long Term Care (CLTC), Sumter County. This is an in‑office role and not a telecommute or remote position.

Social Worker (Lead Team Case Manager)

The Social Worker (Lead Team Case Manager) will promote the mission and values of SCDHHS, work under the general supervision of the Area Administrator as the Lead Team Case Manager, serve as the primary case management quality assurance and compliance reviewer for the local CLTC office, assist in the implementation of a system of case management for Medicaid eligible participants who are receiving community‑based waiver services, provide training and technical support for various Medicaid waivers such as Community Choices and HIV‑AIDS, travel to assigned case locations as necessary, and maintain current knowledge of all CLTC programs and related systems including the Phoenix information system while adhering to HIPAA and privacy laws.

  • Provides oversight, monitoring and maintenance of Area Office Case Management services.
  • Conducts service prior approval process.
  • Performs Quality Assurance and Compliance activities.
  • Provides Area Administrator assistance.
  • Complete waiver enrollment activities.
  • Provide case review related to assessment and service planning.
  • Provide case management services.
  • Perform case assignment activities.
  • Promotes collaboration and effective communication.
  • Performs emergency preparedness duties or activities as required.
  • Performs other duties as assigned.

Qualifications
  • Bachelor’s degree in social work with major coursework in social science program and four (4) years of experience in a social work program; OR
  • Master’s in social work or welfare field accredited by the Council on Social Work Education and two (2) years of experience in a social work program.
Mandatory Special Requirement

Must have current licensure by the SC Board of Social Work Examiners.

  • Requires holder to drive routinely.
  • Valid driver's license.
  • Lifting requirements: 20 lbs.
  • Must have access to a personal vehicle as needed.
  • In‑office role.
Skills and Knowledge
  • Knowledge of service planning and service delivery for persons with disabilities.
  • Skills in assessment, counseling, care planning and case management in long term care.
  • Considerable knowledge of social, mental and cultural aspects of illness.
  • Interpersonal skills for developing and maintaining participant, family and community relationships.
  • Written and verbal skills in evaluating participants’ conditions and needs and interpreting and explaining CLTC program goals to participants, families and other community contacts.
  • Knowledge of HIPAA regulations.
  • Ability to multi‑task within time frames and prioritize cases utilizing effective time management skills.
  • Must be able to perform web‑based software functions (key data, run reports, update documents, etc.).
  • Possess computer proficiency.
  • Gerontological preparation desirable.
