** School Based Support Specialist**!
As a School Based Support Specialist, you will provide intensive outpatient case management and behavioral health support services to youth within school settings. You will work closely with students to develop coping skills, improve social-emotional functioning, and achieve individualized treatment goals while supporting success at school, home, and in the community.
This role involves delivering person-centered interventions, coordinating care with families and service providers, responding to crises, and promoting recovery, resilience, and academic achievement. Services are primarily provided within schools during school hours, with additional support offered in homes and community settings as needed.

Key Responsibilities:
* Provide behavioral health interventions and case management services aligned with individualized treatment plans and goals.
* Deliver services primarily within school settings while supporting youth in home and community environments as needed.
* Assist students in developing coping strategies, emotional regulation skills, social skills, and daily living skills.
* Promote academic success by addressing barriers to school engagement and supporting positive behavioral outcomes.
* Coordinate care with families, schools, community agencies, healthcare providers, and other stakeholders involved in treatment.
* Maintain an assigned caseload and ensure services are delivered in a timely, effective, and person-centered manner.
* Conduct outreach and follow-up activities, including home visits and engagement efforts for missed appointments.
* Provide crisis intervention, de-escalation, and follow-up support as appropriate.
* Complete accurate and timely documentation in the Electronic Health Record (EHR) and other required systems.
* Participate in treatment planning, team meetings, supervision, and interdisciplinary collaboration.
* Assist with quality assurance activities, data collection, program evaluation, and compliance with agency and contractual requirements.
* Support program goals through flexibility, collaboration, and responsiveness to evolving student needs.
* Participate in ongoing professional development and training opportunities.

Education and/or Experience
Qualifications:
Candidates must meet one of the following qualifications:
* Bachelor's degree in Psychology, Social Work, Behavioral Health, Counseling, Human Services, or a related field; OR
* Bachelor's degree in an unrelated field with two years of related behavioral health experience; OR
* Any combination of four years of higher education and two years of qualifying behavioral health experience; OR
* Associate's degree in Applied Science in Behavioral Health Support; OR
