Intermountain Health is hiring a Social Work Care Manager to collaborate with physicians and manage care for patients with chronic conditions. This role is fully remote but may require onsite travel. Candidates must have an MSW and a current LCSW license.
The ideal applicant will demonstrate critical thinking and communication skills and work effectively in interdisciplinary teams to promote positive patient outcomes.
Salary range is $35.60 - $54.93 per hour depending on experience.
