Summary
The Social Worker in this position provides direct services to adult and geriatric veterans enrolled in the Home Based Primary Care (HBPC) Program with high quality case management, clinical treatment, advocacy, and coordination of care with appropriate VA and community service providers/agencies as needed by the client.
Responsibilities- Conduct comprehensive psychosocial assessments and develop treatment plans in collaboration with interdisciplinary teams.
- Provide case management, crisis interventions, and advance directive education.
- Advocate for veterans and coordinate community resources, VA programs, and entitlements.
- Document assessments, interventions, and progress in the electronic health record.
- Maintain up‑to‑date knowledge of medical and mental health diagnoses, treatments, and community resources.
- Supervise or provide mentorship to new social workers and graduate students.
- Serve as a subject‑matter expert on VA and community resources and assist other staff with psychosocial needs of patients.
- United States Citizenship; non‑citizens may be appointed only if no qualified citizens can be recruited.
- Master’s degree in social work from a school accredited by the Council on Social Work Education (CSWE).
- Doctoral degree in social work is not a substitute for the master’s degree.
- State licensure or certification to independently practice social work at the master’s level.
- Minimum one year of post‑MSW experience equivalent to the GS‑9 grade in health care or related settings, with independent practice licensure.
- Proficiency in spoken and written English as required by 38 U.S.C. § 7403(f).
- Knowledge of community resources, medical and mental health diagnoses, and treatment procedures.
- Skill in conducting psychosocial assessments, implementing treatment modalities, and collaborating with interdisciplinary teams.
- Work schedule:
Monday – Friday, 8:00 am – 4:30 pm. - Competitive salary with regular increases.
- Paid time off: 37‑50 days annually, inclusive of sick leave and federal holidays.
