POSITION SUMMARY

Case management positions consist of advocating and assisting participants with waiver services.

DUTIES AND RESPONSIBILITIES
  • Conduct annual assessment and person-centered service plan.
  • Conduct monthly face-to-face visits with participants.
  • Assist clients with accessing community resources.
  • Monitor participants’ progress with waiver services including health, safety and welfare in monthly contact notes.
  • Assist with building a network of supports.
  • Assure participant choice and access to community resources.
  • Return phone calls and emails promptly.
  • Complete and submit documentation by required deadlines.
  • Perform other related work as required and assigned.
QUALIFICATIONS
  • BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences.
  • One year of experience working with /DD individuals (e.g., volunteering or having a relationship with family or friend who may have /DD) or one year of case management experience.
  • Valid driver’s license and vehicle insurance required.
  • Strong organizational skills.
  • Ability to multi-task.
  • Excellent communication skills.
  • Self-motivated.
  • Ability to conduct and facilitate meetings.
  • Ability to collaborate with other team members.
  • Ability to advocate for the needs of the participant.
JOB REQUIREMENTS / EXPECTATIONS
  • This job consists of sitting to complete necessary paperwork on computer.
  • Must be able to operate a motor vehicle and have reliable transportation with valid insurance.
  • Must provide verbal and written communication with team members.
  • Job setting is at the corporate office and/or home office setting.
  • Documentation is time sensitive and must follow waiver regulations.
  • Work schedule is Monday‑Friday 8am‑5pm. Schedules can vary depending on participant visits. Flexible working hours.
