Case Manager
Case management positions consist of advocating and assisting participants with waiver services.
DUTIES AND RESPONSIBILITIES- Conduct annual assessment and person-centered service plan.
- Conduct monthly face-to-face visits with participants.
- Assist clients with accessing community resources.
- Monitor participants’ progress with waiver services including health, safety and welfare in monthly contact notes.
- Assist with building a network of supports.
- Assure participant choice and access to community resources.
- Return phone calls and emails promptly.
- Complete and submit documentation by required deadlines.
- Perform other related work as required and assigned.
- BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences.
- One year of experience working with /DD individuals (e.g., volunteering or having a relationship with family or friend who may have /DD) or one year of case management experience.
- Valid driver’s license and vehicle insurance required.
- Strong organizational skills.
- Ability to multi-task.
- Excellent communication skills.
- Self-motivated.
- Ability to conduct and facilitate meetings.
- Ability to collaborate with other team members.
- Ability to advocate for the needs of the participant.
- This job consists of sitting to complete necessary paperwork on computer.
- Must be able to operate a motor vehicle and have reliable transportation with valid insurance.
- Must provide verbal and written communication with team members.
- Job setting is at the corporate office and/or home office setting.
- Documentation is time sensitive and must follow waiver regulations.
- Work schedule is Monday‑Friday 8am‑5pm. Schedules can vary depending on participant visits. Flexible working hours.
