A Day in the Life of a Resource SpecialistIn this position, the Resource Coordinator provides person‑centered support to older adults and caregivers by completing in‑person and phone assessments, offering options counselling on long‑term care services, benefits, housing, and financial resources, and assisting individuals and families with care planning and transitions following hospital, rehabilitation, or nursing facility stays. The role involves coordinating services and referrals with healthcare providers and community agencies, advocating for clients to ensure access to appropriate supports, and managing an ongoing caseload with accurate documentation of assessments, plans, and services to promote successful community living.
Qualifications- Bachelor's degree in social work, gerontology, nursing, or a related human service field. Licensed Social Workers preferred.
- 2 years of professional experience with case management, care coordination, and discharge planning in a long‑term care, hospital, or community setting.
- Knowledge of aging services, community resources, and long‑term supports.
- Reliable transportation.
- A heart for working with the elderly.
This position works Monday – Friday, 8:00 AM – 4:30 PM, with two in‑office days per week and the remaining time spent working from home or in the community. When working offsite, employees must meet specific internet criteria, including demonstrated high‑speed connectivity and the ability to meet privacy and security standards with secure (password‑protected) internet and demonstrate successful performance.
