Trinidad: Care Coordinator

People Care Health Services is hiring a Care Coordinator to manage client caseloads and coordinate quality home care services across various counties in Southern Colorado
. This role includes assessments, client follow‑up, caregiver coordination, scheduling support, documentation, and communication with families, providers, and partner agencies to ensure care is delivered effectively and compassionately.

• Manage an assigned caseload of clients
• Coordinate care with clients, families, caregivers, and community partners
• Complete intake assessments and required client visits
• Support caregiver scheduling and coverage needs
• Help address client or staff concerns promptly and professionally

• Healthcare, case management, or caregiving experience preferred
• Strong communication, organization, and problem‑solving skills
• Comfortable working independently, in a fast‑paced environment, and as part of a team

This role includes working from home, attending occasional in‑office meetings, visiting client homes, and traveling throughout the field. Travel is required up to 50% of the time.

Salary Range: $20.71 To $24.00 Hourly
