Bluestone Physician Services is seeking a Care Coordinator to serve Minnesota seniors living in the community and assisted living facilities. You will work from home, traveling locally to meet member needs and coordinate DHS and CMS required activities.
This full-time, day-shift role requires 8am-5pm Monday through Thursday and 8am-3pm on Fridays, with 50-70% travel in the Winona area and 30-50% remote work; salary range $65,000-$75,000 depending on experience.
