Bluestone Physician Services is seeking a Care Coordinator to support Minnesota Senior Health Options (MSHO/MSC+) members living in the community and assisted living facilities. You will work from home, with regular local travel to meet member needs and support the community.
The role is full-time with day-shift hours; travel is ~50–70% in the Maplewood area and ~30–50% remote. Responsibilities include visits, assessments, care planning, and coordinating with care teams.
