The Case Care Manager will also support non-SUD Project individuals seeking legal assistance from NNJLS.

This role will provide leadership, client support, quality control, assistance with grant compliance oversight, and social work program development within a nonprofit legal services setting. The Case Care Manager will integrate social work services into legal advocacy, supervise a Client Navigator, support NNJLS case handlers and staff, build partnerships with treatment providers and community agencies, and help integrate strong social work practices into NNJLS’ legal advocacy model.

Key Responsibilities

  • Provide consultation, complex case support, direct client intervention, care plan development, resource coordination, and referrals as needed.
  • Develop and maintain strong referral networks with SUD providers, recovery programs, hospitals, housing providers, county agencies, and community partners.
  • Work closely with NNJLS case handlers and other staff to identify social, behavioral health, housing, financial, and family-related barriers impacting client stability.
  • Support program development, workflows, referral protocols, documentation standards, and service coordination processes with a focus on Social Work practice.
  • Monitor quality control for client navigation, referrals, follow-up, documentation, and service outcomes.
  • Assist with grant compliance and reporting, data tracking, and regional expense/program activity documentation.
  • Ensure services are trauma-informed, culturally responsive, recovery-oriented, and aligned with best practices for working with individuals with SUD.
  • Supervise and coordinate Social Work interns, including onboarding, task assignment, coaching, and performance feedback.
  • Travel regularly throughout Bergen, Passaic, and Hudson Counties, and other service locations as needed to support client services, outreach, and partner engagement.
Qualifications
  • Master’s Degree in Social Work preferred.
  • Current New Jersey social work credential (LSW) required; LCSW preferred. Candidates with significant relevant experience and an active NJ LCADC credential may also be considered.
  • Significant case management, behavioral health, substance use, healthcare, nonprofit, or community-based experience.
  • Prior supervisory, leadership, or program coordination experience required.

Minimum of 5 years’ experience working with individuals with substance use disorders, individuals in recovery, or vulnerable populations strongly preferred.
  • Ability to supervise staff and Social Work interns.
  • Strong knowledge of New Jersey social service systems, housing resources, public benefits, recovery supports, and community-based services.
  • Experience with grant compliance, quality assurance, documentation review, and reporting preferred.
  • Strong communication, collaboration, organizational, and problem-solving skills.
  • Ability to work effectively in a legal services environment with NNJLS case handlers and staff, clients, and external partners.
  • Ability to travel regularly across NNJLS offices and partner locations.
