Location: City of Albany
Northern Rivers Family of Services is seeking a Clinical Time Transition Program (CTTP) Youth Peer Advocate to support youth transitioning from hospital settings back to their communities. You will work with youth, families, schools, and community resources to promote reintegration and long-term stability, drawing on your lived experience to empower self-advocacy.
You will report to the Director of the Hospital to Home program and may work onsite with travel and occasional remote options.
