Role Description The Hospital 2 Home Social Worker is a full-time Monday - Friday, hybrid role based in the Las Vegas Metropolitan Area. This role focuses on supporting older adults and individuals with disabilities during transitions from hospital to home, helping to reduce readmissions and promote safe, sustainable community living.
Day-to-day responsibilities include conducting psychosocial assessments, coordinating discharge plans, connecting individuals and caregivers to community resources, and developing individualized care plans. The Social Worker will collaborate closely with hospitals, healthcare providers, and Nevada Senior Services programs, provide ongoing follow-up and case management, and advocate for client needs. Documentation of services, maintenance of accurate records, and participation in multidisciplinary team meetings are also key aspects of this position.
Qualifications
- Professional social work skills, including assessment, case management, and care planning, with experience supporting older adults and individuals with disabilities.
- Strong communication and interpersonal abilities to collaborate with clients, caregivers, healthcare teams, and community partners.
- Organizational and time-management skills to handle multiple cases, maintain detailed documentation, and meet program deadlines.
- Knowledge of community resources, healthcare systems, and transition-of-care best practices, particularly in aging and caregiving services.
- Ability to work effectively in a hybrid environment, including in-home and hospital visits as well as remote work, with reliable transportation within the Las Vegas Metropolitan Area.
- A Bachelor’s or Master’s degree in Social Work or a closely related field and licensure for social work licensure in Nevada (LSW or LMSW). For those with MSW, 3 years of case management experience is required.
- Commitment to person-centered, culturally responsive, and non-discriminatory practice, with sensitivity to diverse backgrounds and needs.
- Experience with electronic health records or case management systems and basic proficiency with standard office software.
