Position Summary
The Case Management Coordinator is a work-from-home position with travel required (in Pima County, Arizona) to visit members. This role coordinates, documents, and communicates all aspects of the utilization/benefit management program, applying critical thinking and medical necessity criteria to provide care coordination, support, education, and coaching.
Responsibilities- Conduct comprehensive evaluations of referred members’ needs and eligibility, recommend case resolution approaches based on benefit plans and available programs.
- Identify high‑risk factors and service needs, coordinate and implement assigned care plans, and monitor progress.
- Use a holistic approach to overcome care barriers, present cases at multidisciplinary conferences, and identify quality‑of‑care issues.
- Negotiate appropriate options and services, employ motivational interviewing skills, and promote member engagement and lifestyle changes.
- Coach members to make independent medical and lifestyle choices and support collaboration with providers in decision‑making.
- Maintain case‑management documentation in compliance with regulatory and accreditation guidelines and company policies.
- 2+ years of experience in case management with individuals having serious mental illness (SMI) or elderly/physically disabled.
- Must reside in Pima County, Arizona.
- Willingness to travel up to 50% within Pima County.
- Strong organizational and time‑management skills.
- Ability to collaborate with internal and external partners.
- Proficiency in Microsoft Office Suite.
- Previous collaboration with medical professionals.
- Bilingual (English/Spanish).
- Bachelor’s degree in Social Work, Psychology, Special Education, or Counseling.
- Licensed Registered Nurse (RN) valid in Arizona.
Pay range: $21.10 - $40.90 per hour (or equivalent salary). Base pay depends on experience, education, and geography. Eligible for bonus, commission, or short‑term incentive programs.
