About the Role
SCAN is a nonprofit health organization dedicated to improving care for older adults. The Community Health Worker role enhances frail seniors’ ability to age in place and manage health by providing person‑centered care in accordance with ECM requirements. The position is based in Orange County, CA and requires both remote and in‑field work.
Key Responsibilities- Collaborate with members via phone and in‑person to implement care plans, advocate on behalf of members, and provide member‑centric interventions.
- Conduct general assessments to identify chronic conditions and psychosocial challenges affecting independent living.
- Perform assessments for Medi‑Cal/Medicare dual‑eligible members referred to Complex Care Management for LTSS and assist the RN in determining nursing facility level of care.
- Support member well‑being through observation, positive communication, and motivational interviewing.
- Conduct face‑to‑face or phone visits to address care barriers, ensure engagement in primary and preventative care, and support individualized care plan implementation.
- Provide general care coordination and empower members and families in managing chronic conditions through coaching, education, health‑care navigation, advocacy, accompaniment to appointments, and referrals to community, county, and state resources.
- Perform ECM activities for specific populations of focus, including individuals experiencing homelessness, at‑risk for avoidable hospital/ED utilization, adults at risk of LTC institutionalization, and nursing facility residents transitioning back to the community.
- Implement personalized care plans for high‑risk members, incorporating realistic health goals that support members’ wishes.
- Participate in interdisciplinary planning and case conferences to ensure person‑centered care and follow‑up after discharge.
- Maintain timely, accurate documentation of care plans, service plans, and progress notes per SNP policies.
- Build relationships with community organizations and identify resources for members.
- Follow departmental procedures, workflows, and training materials; report barriers to supervisors.
- Assist with basic problem‑solving and refer complex issues to higher‑level staff.
- Travel up to 50% of the time within Orange County and surrounding areas.
- High School Diploma or equivalent.
- Community Health Worker Certificate strongly preferred.
- Bilingual in English and Spanish (proficiency test administered if required).
- Minimum 1 year of community engagement experience, preferably with seniors, home visits, and remote work.
- Experience in managed care, health‑care environment, lived experience or case management strongly preferred.
- Basic technical, problem‑solving, communication, organizational, and critical‑thinking skills.
- Ability to collaborate within a multidisciplinary team and maintain confidentiality.
- General understanding of NCQA standards, CMS and DHCS regulations, and basic medical terminology.
- Valid driver’s license, automobile insurance, and reliable transportation.
Proof of recent tuberculosis screening is required upon hire.
