Location: Town of Vermont
The Adoption Program at Lund seeks a Post Adoption Case Manager dedicated to supporting families formed through adoption. The role focuses on family stability through parent coaching, trauma education, and advocacy, with home visits and community resource connections.
This is a home-based, 40-hour-per-week position requiring in-person work in Windsor and Windham Counties, VT, with regular travel in southern Windsor County.
