The Case Manager is responsible for supporting individuals with disabilities in developing and implementing person‑centered plans that promote independent living. Day‑to‑day tasks include conducting intake assessments, identifying needs and goals, coordinating services and resources, and advocating on behalf of participants with community agencies and service providers. The role involves maintaining accurate documentation, tracking progress, and providing ongoing follow‑up to ensure services remain responsive to each person’s choices.
The Case Manager will collaborate with internal teams and external partners to connect individuals to housing, employment, benefits, and skills training. This is a full‑time, hybrid role based in St Paul, MN, with a combination of on‑site work and some work from home.
- Strong case management and service coordination skills, including assessment, planning, and follow‑up.
- Knowledge of disability rights, independent living philosophy, and community resources that support people with disabilities.
- Effective communication and interpersonal skills, including the ability to build trust, listen actively, and work with diverse communities.
- Organizational and documentation skills, including maintaining accurate records and managing multiple cases and deadlines.
Ability to work collaboratively in a team environment and independently in a hybrid setting. - Experience in social services, human services, rehabilitation, or a related field; prior experience working with people with disabilities is highly beneficial.
- Basic proficiency with office software and databases used for case management and reporting.
- Relevant education such as a bachelor’s degree in social work, human services, psychology, or a related discipline, or equivalent combination of education and experience.
