Overview
A Case Manager at the Oregon Department of Human Services coordinates long‑term care services for Oregonians, develops individualized case plans, and ensures eligibility and safety across in‑home, community‑based, and nursing home settings.
Responsibilities- Assess applicants and current service users to develop, coordinate, and arrange a variety of services to enable continuity of care for older adults and people with disabilities.
- Provide choice counseling, develop and implement case management plans, and coordinate the provision of services in accordance with rules, regulations, and individual case plans.
- Contact Oregonians or collateral contacts monthly—by phone, in person, or electronically—to review and update service plans and safety needs.
- Ensure services are delivered in a culturally competent, trauma‑informed manner, with printed materials available in multiple languages and formats and bilingual services where required.
- Collaborate with the eligibility team and utilize the ONE System to review and update Long Term Care Services and provide information to Oregonians.
- Interview Oregonians to determine initial and ongoing eligibility for health, social, and protective services, including collateral interviews both in person and by phone.
- Develop detailed, individualized, person‑centered case plans using the Case Assessment and Planning System (CA/PS) on a state‑issued laptop.
- Process applications, inquiries, and data in accordance with policy timelines, ensuring appropriate documentation and communication of eligibility decisions.
- Work with Adult Protective Services, AFH Licensors, and other agencies to ensure the safety and protection of Oregonians and follow mandatory reporting guidelines.
- Three years of human services related experience; OR a bachelor’s degree in Behavioral Science, Social Science, or a closely related field; OR an associate degree in the same field plus two years of human services experience; OR an equivalent combination of experience, education, and training.
- Experience coordinating services and supports by assessing needs, developing service plans, monitoring progress, and adjusting services to promote positive outcomes.
- Experience interpreting and applying laws, regulations, policies, or procedures to make consistent, accurate, and well‑supported decisions.
- Experience preparing clear, accurate, and timely documentation that supports decisions and complies with organizational or legal requirements.
- Experience conducting interviews or information‑gathering conversations to assess needs and determine appropriate next steps using sound judgment.
Full‑time schedule: 8:00 a.m. – 5:00 p.m., Monday–Friday PST, with occasional overtime. Work occurs in an office and community settings, with remote work possible upon managerial approval. A flexible schedule may be required. Travel may be necessary. A valid driver’s license is required. ADA accommodations may be provided. The role involves interacting with individuals who may have experienced trauma and requires reliable attendance and the ability to use computers and technology.
