The Ohio Children's Alliance in Northwest Ohio is seeking a Case Coordinator for the Bridges program. You will report to the Program Manager and organize transition services for youth aging out of foster care, coordinating with PCSA, IL teams, and community partners.
The role requires strong coordination, communication, and relationship-building with stakeholders to ensure smooth Warm Handoff processes, housing support, and eligibility for Bridges.
